Medical Network One, a Rochester, Michigan-based physician services organization and health management service provider offering comprehensive disease management prevention and support for individuals living with chronic conditions, has promoted Margaret Kucinski, RN, BSN to the newly created position of Director of Clinical Services. The announcement was made by Ewa Matuszewski, CEO of Medical Network One.
"Margaret has been with Medical Network One since 1986 and has played a pivotal role in implementing our disease management programs and health care initiatives," says Ms. Matuszewski. "Her clinical and administrative talents will further benefit the organization as we increase our efforts and focused resources on the Patient-Centered Medical Home and the Community Care Travel team.
The Community Care Travel Team (CCTT) is a multi-disciplinary team of health care specialists, including a registered nurse, certified diabetes educator, registered dietitian, behavioral health specialist, and exercise specialist. The CCTT brings its combined expertise to physicians' offices, the workplace, schools and community centers to evaluate and provide counseling to individuals with targeted chronic conditions including: diabetes, asthma, obesity, congestive heart failure, coronary heart disease, depression and lower back pain.
Ms. Kucinski helped to implement the Community Care Travel Team in 2006 in Medical Network One physician member offices and continues to oversee the program in its current expansion phase. She has also played a key role in Medical Network One's efforts to educate physicians and their practice teams on the Patient Centered Medical Home (P-CMH). The P-CMH is a primary care movement that advocates a team-based approach to providing health care. In the P-CMH, a partnership is developed between the patient, his or her primary care provider and a health care team; working together, care and services are coordinated to provide optimal patient care.
In addition to her experience at Medical Network One, Ms. Kucinski's career history includes serving as a hospital staff nurse, a nursing instructor and a family practice nurse. She is a member of the American Association of Diabetic Educators, the Disease Management Association of America and the Case Management Society of America and is certified in Managed Care Nursing.
Ms. Kucinski holds a bachelor's degree in nursing from Grand Valley State University. She is a resident of Rochester Hills.
